Why You May Notice a Few Changes
As Open Table KC welcomes more volunteers and community partners, we have an opportunity to do more than serve dinner each Tuesday. By building friendships, earning trust, and having meaningful conversations, we can better understand our neighbors’ needs and support them as they work toward their goals.
Volunteer check-ins and simple outreach records help us track attendance, meals served, supplies distributed, needs identified, and the full impact of our work. Strong, accurate data also improves our ability to apply for grants and funding that could be life-changing for the community we serve.

Use this form to record attendance for a returning community member at Tuesday outreach.
Additional Outreach Forms
Use this form when someone attends Tuesday outreach for the first time. After registering them, return to the Community Member Check-In form to record their attendance.
Use this form when you have a conversation with a Community Member that reveals a possible need for assistance or resources, or when you personally observe something that may warrant follow-up, even if you do not feel comfortable approaching the person directly.
Use this form to record supplies or essential items distributed during outreach, including the item and quantity provided.
